The following is a list of things that we will need from you to get started.
First, you will need to go to the Contact us tab and let us know how to contact you. After we get your contact info you will get a welcome packet and some forms. Once you receive the packet and have reviewed the material we will need:
1) Information about your practice.
a) Practice Locations and hours of operation
b) Pay to address
b) Phone and fax numbers
d) Tax ID
e) NPI number
f) List of insurance panels currently participating in
2) Your Client Data
a) Client registration forms.
1. Client Name
2. Date of Birth
4. Insurance Company with Client Id and Group numbers.
5. Client Signature to contact/release info to their insurance
company (very important!)
3) Daily Log Sheets
Daily Log Sheets are used for tracking your client sessions and monies paid by your client. You simply just fill out the form and fax into us daily or weekly.
4) Billing Agreement
In our welcome packet you will also receive a copy of our billing agreement. This includes our commitments to you as a provider as well as your commitments to us as your billing service. You may sign our agreement and either mail or fax it back to us.
After you have signed our Agreement you will be assigned a billing specialist. Communication with the billing specialist is done through phone, fax and email. I have both full and part time staff, but require that all employees must work at least 4-5 days a week for your convenience and for consistency.
